This double-sided Emergency Communications Planning Card is designed to help members of a houshold plan for a disaster. One card is required for each household member. Each Emergency Communications Planning Card comes with a top part that contains a household member's critical contact, medical, and insurance information (for storing with your emergency kit), and a lower portion (the Wallet Card) that contains your emergency contact information and should be kept in your wallet or purse at all times.

Emergency Planning Card Instructions:
The members of your household may not be together when disaster strikes, so use this card to help you plan how to contact each other and review what you will do in the event of an emergency. Complete both sides of this card (one card for each family member). Store the top portion of all cards with your emergency kit. In addition, remember to tear off the wallet card and keep it with you at all times. Make sure that the information on your wallet card is the same for all family members.
